Diagnostic records are needed to fully diagnose problems and determine the specific treatment. This consists of:
|
- Panoramic x-ray: one continuous image of the teeth and jaws
- Cephalometric x-ray: an image of the whole head
- Clinical exam
- Photos: Photos give close-up views of the front and sides of your bite. Pictures of your teeth may be taken with your mouth open and closed. The front and sides of your face may also be photographed. These pictures show how your bite problem affects your appearance.
- Study models: models show how your top and bottom teeth meet

The charge for your records will be $270.1 Payment is due the day we do the Diagnostic records. This appointment is about an hour and will need to be made in the morning.

Treatment plan
Based on your diagnostic records, Dr. Dilley will put together a Treatment Plan for you. He will meet with you three weeks after these records are taken. Dr. Dilley will go over what your treatment will consist of and he will be able to give you a price on the treatment.2 After you have your 30 minute treatment plan with Dr. Dilley, we can help you find any additional doctors you may need to see for other work that may be required before starting your treatment.

Insurance
If you have orthodontic insurance, our office will file your claim for you. Please obtain a Dental Insurance Form from your employer and complete the top part of the form, and bring it to your Records appointment. We will also file a pre-estimate for the cost of treatment once a price is determined. All insurance reimbursements will be mailed directly to the patient.

Payment plan
When you are ready to begin treatment, Dr. Dilley requires a 25% down payment. The staff will work with you to set up monthly payments with no finance charges. There are several options:
|
- Put your Visa or MasterCard number on file and charge your credit card. We will send you a receipt by mail every month. We can also send you a ledger sheet for your flex spending account at your request.
- Coupon book with mailing envelopes for sending a monthly payment by check.
- You can arrange with your bank to have a check sent each month.
- Pay by credit card or check when you come in for appointments.

XXAll payments are due on the 25th day of each month.

1 If you have one child already in treatment, your second child’s Diagnostic Records are FREE. That’s a savings of $270.
2 If you decide to have Dr. Dilley provide your orthodontic treatment, he will deduct $150 from your total treatment cost.
